We do not participate with Star Medicaid, Pacificare/Secure Horizons, Tricare or Humana.

Traditional Medicaid patients require a referral from their primary care physician.

If you have any questions about whether or not we participate with your plan, please call the office or your insurance company to verify.

We only file claims with your primary insurance company; you will need to file for any secondary or tertiary coverage you may have. We will provide you with a coded receipt to attach to the EOB (Explanation of Benefits) from your primary carrier.

Co-payments, co-insurance, and/or deductible amounts are payable at the time of service. Any balance remaining after insurance has paid is due within 30 days unless you contact our administrator to arrange reasonable payment terms.

You must do the following for your initial visit:

1). Verify that we are participating with your insurance plan.

2). Be sure your referral has been provided to us by your PCP (primary care physician), or bring it with you to your appointment.

3). Be in possession of your current insurance card and a valid driver's license or other picture identification card (ie. military I.D. card or state issued I.D.).

4). Bring all relevant medical records, x-rays, MRI's, CT Scans, etc. with you so we do not have to reschedule your appointment.

STOSSA also treats patients that have been injured at work or in an automobile or other third-party liability type accident. Please call the office and speak to our administrator for specific details regarding these types of injuries.
